This is part of what came back in the mail when we applied for Medicaid for Josh:
Problems
How do we get an "FL-2" or "MR2 and plan of care"? What are they?solved; FL-2 ready to deliver to Medicaid (form is triplicate and apparently not available online)- How can we get proof of income for a nonverbal autistic minor with no income?
- How can we get bank account numbers and bank consent/release forms for someone with no bank accounts?
- How can we get copies of a life insurance policy for someone with no life insurance?
- We applied for social security disability for Josh, but he didn't qualify because Sandy has too much money in her emergency stash (we probably wouldn't have been able to get the mortgage if she hadn't had them, but they won't count the debt on the mortgage against the emergency stash) Tiffany at Maxim is investigating this

2008-04-24 update: Tiffany at Maxim Healthcare is looking into these remaining issues, especially the SSD
